Plumbum metallicum is prepared from pure metallic lead. Historically, lead has been used in plumbing, paints, glazes, cosmetics, ammunition, and industrial materials, though now largely restricted due to its known toxicity. In toxicology, it produces progressive paralysis, sclerotic degeneration of nerves, muscles and tissues, and intense constipation with abdominal retraction. In homeopathy, it is a deep-acting antipsoric with profound effects on the nervous system, musculature, gastrointestinal tract, and mind, with marked tendencies to constriction, sclerosis, and retraction.

It suits those in advanced chronic states, often with sclerosis, calcification, or neurological decay. The patient resists change and isolates within self-constructed walls of fear, rigidity, and internal pressure.

Symptoms

Mind

Plumbum metallicum produces a state of mental dullness, sluggishness, and suspicion. The mind becomes slow to grasp ideas, forgetful, unable to remember words or names [Kent]. A heavy, paralysed slowness affects all thinking. Apathy and indifference dominate. However, this is often mixed with deep suspicion, delusions of persecution, and monomania—a fixed idea obsessively clung to. There is a sense of internal retraction, as if the world is receding, and the mind withdraws from stimuli. Melancholia with suicidal ideation may occur, especially in degenerative neurological conditions. In children, there may be lack of development, obstinate behaviour, or mental retardation. Fear of being poisoned or harmed. Delirium may alternate with idiocy.

Sleep

Restless, disturbed by cramps or nerve pain. Cannot find a comfortable position. Sleepless from worry or physical tension. May fall asleep suddenly, but with unrefreshing quality. Dreams of pain, mutilation, or effort.

Dreams

Vivid, oppressive dreams. Dreams of suffocation, paralysis, being trapped or bound. Nightmares of losing strength or faculties. Dreams of mechanical entrapment or being crushed.

Generalities

A remedy of retraction, paralysis, sclerosis, and constriction. Acts deeply on the nervous system, gastrointestinal tract, and muscles. Better from pressure, worse from motion. Progressive degeneration, especially in chronic conditions. Characteristic drawn-in sensation, as if tendons or organs were shrinking. Suitable for slow, cold, suspicious, aged, or withered constitutions.

Fever

Chill predominates. Coldness with dry heat in isolated areas. Fever in low-grade inflammatory conditions or uraemia. Face flushed, but extremities cold. Intermittent fever patterns. Dry skin despite fever.

Chill / Heat / Sweat

Chill with trembling. Heat in head or chest only. Sweat scanty or absent. Offensive odour when present. Sweat cold, clammy. May occur during colic.

Head

Headache with pressing pain in temples and vertex, better from firm pressure. Drawing, tightening sensations, as if the brain were pulled back or inward. Head feels heavy, especially at the occiput. Forehead may be cold. Hair may fall out. Face pale, with anxious or sunken expression. Cerebral atrophy and sclerosis are key themes. Head may be drawn backward in spasms.

Eyes

Pupils contracted, sluggish or unequal. Optic nerve atrophy with gradual blindness. Diplopia, strabismus, or ptosis may be present from nerve palsy. The eyes may feel as though protruding or drawn back. Vision dim or cloudy. Flickering before the eyes. Eyes sunken. Photophobia with tearing. Ophthalmoplegia in advanced cases.

Ears

Noises in ears—buzzing, ringing. Hearing impaired or lost due to auditory nerve involvement. Sensation as if ears were closed. Ears pale or bluish. Pressure or tension behind ears. Otalgia extending into mastoid.

Nose

Dryness of nostrils. Loss of smell. Frequent sneezing with tingling. Sensation of retraction or tightness in the root of the nose. Epistaxis possible in elderly or sclerotic constitutions.

Face

Face pale, earthy, or sallow; sunken cheeks and hollow eyes. Bluish circles around the eyes. Lead line on gums (bluish-black margin). Lips dry, cracked. Twitching or paralysis of facial muscles. Expression anxious or sad. Trismus or tetanic tension of jaw may be present in neurological cases.

Mouth

Dryness of mouth, especially at night. Metallic or bitter taste. Gums swollen, bleeding, blue-edged. Tongue coated white or yellow; trembles when protruded. Speech slow or hesitant. Paralysis of tongue in severe cases. Ulcers may form in mouth due to poor circulation.

Teeth

Teeth feel elongated or loose. Painful on biting. Toothache worse from cold drinks or touch. Gums spongy and retracted. Metallic lead line around teeth.

Throat

Chest

Heart

Palpitations violent, with oppression. Pulse hard, small, irregular. Tendency toward arteriosclerosis and high blood pressure. Sensation as if heart were held tight in a vice. Cardiac weakness or degeneration possible.

Respiration

Slow, shallow, laboured. Intermittent or gasping respiration. Breath may be cold. Worse on exertion or at night. Difficulty expanding chest. Threat of paralysis of respiratory muscles in severe cases.

Stomach

Marked gastric constriction and spasms. Cramping pain worse after food. Appetite may be lost, or ravenous with weight loss. Nausea with retching, but vomiting rare. Sensation of a string pulling the stomach backward. Gastric ulcers in chronic toxicity. Stomach pain radiates to the spine.

Abdomen

One of the chief spheres of action. Classic “lead colic” presents with violent twisting, cramping pain, causing the patient to bend double or press the abdomen. Sensation as if the abdominal wall were being drawn inward. Abdomen hard and retracted, sometimes forming a concave surface [Clarke, Hering]. Constipation accompanies this state. Tender to touch. Gurgling or rumbling absent. Pain radiates in all directions. Mesenteric glands may be indurated.

Rectum

Constipation is extreme, with paralytic inertia of the intestines. No urge, or constant ineffective urging. Stools dry, hard, in balls, often adhering to the rectum. Rectum feels closed. Tenesmus without result. Haemorrhoids may be present, purple, and painful. Occasional involuntary stools in paralytic states.

Urinary

Urine scanty, dark, albuminous. Suppression of urine in nephritis or lead intoxication. Tenesmus vesicae. Pain in urethra on urinating. Incontinence or dribbling from paralysis of bladder sphincters. Odour strong. Pressure in kidneys and pelvis. Uraemia possible in advanced cases.

Food and Drink

Aversion to meat and fatty food. Thirst increased during pain. Craving for sour or pungent things. Aggravated by alcohol, coffee. Digestion impaired, but appetite variable.

Male

Impotence with cold, shrunken genitals. Emissions involuntary. Testes feel drawn up or aching. Prostate enlarged. Gonorrhoea suppressed with bad after-effects. Seminal fluid scanty or absent. Weakness post-coitus.

Female

Menses suppressed or delayed. Uterine constriction, as if tightly bound. Labour-like pains in non-pregnant women. Pain extends from uterus to thighs or sacrum. Ovarian pain with induration. Breasts hard and nodular. Sterility from uterine sclerosis. Leucorrhoea thick, tenacious.

Back

Spine stiff, painful, or sensitive to pressure. Drawing, retracted sensations. Paralysis ascending from lower limbs, affecting lumbar spine. Weakness and numbness of dorsal spine. Better from hard pressure. Coldness down spine.

Extremities

Profound weakness, paralysis, atrophy, and retraction dominate this remedy. “Wrist drop” is a keynote [Boericke]. Arms or legs feel drawn inward. Limbs cold, emaciated, and bluish.
